Customer Service Record (CSR)

The customer service record (CSR) is details the legal ownership attributes of a phone number. This information details how the phone number record appears in database.

Use existing record optional)

To populate the CSR with the name and caller ID information that’s already being used on another phone number on the account, expand Use Existing Record and select the record you want to use.

Skip this step to create a new record.

Individual or Company*The tab selected here indicates whether this phone number record is for a company or an individual. This setting cannot be changed later.
NameThe first and last name of the individual or the name of the company this number is assigned to.
Caller ID nameThe name displayed when this number calls another number. This field is automatically populated with information from the Name fields, but can be edited.
Address same as above
Check this box to indicate the information for the Customer Service Record (CSR) is the same as the information provided under Select a Number above.

If the information is different, make sure this box is unchecked, and fill out the fields below.
Postal codePostal or ZIP code of the address on the CSR.
Enter addressThe address that will be associated with the phone number being added. Some addresses will require defining a unit type and number, such as an Apartment. These fields will become available and show as required under the Manual Entry section.
Manual Entry

If you want to manually enter the address in a granular format, click here. For example, if the address is for a multi-unit building, you can add the Unit Type and Unit Number here. These fields will automatically populate with the information previously provided and are optional unless otherwise indicated.
